What is CCSP?
The Community Care Services Program (CCSP) helps people who
are elderly and/or functionally impaired to continue living in their homes and
communities. For elderly and/or functionally impaired people, CCSP offers
community-based care as an alternative to nursing home placement.
The Division of Aging Services, a part of the Department of Human Resources,
administers CCSP.
Who
is eligible for these services?
CCSP offers community-based services to persons who:
- Meet the functionally impaired criteria when assessed by a
local assessment team. The assessment team works with individuals and their
families in developing plans of care which are approved by the individual's
physicians
- Would otherwise be moving into a nursing home
- Are eligible or potentially eligible for Medicaid
- Can be provided appropriate community services within state
costs limits
The assessment teams helps an individual determine
appropriateness for the program. A local case manager links eligible persons to
community services.
What services are available?
Emergency Response System (ERS)
An in-home electronic support system which provides two-way verbal and
electronic communication to a geographically and social isolated individual. ERS
provides the individual a means of communication and monitoring 24 hours a day.
Home Delivered Meals (HDM)
Meals that provide at least one-third of the recommended daily nutritional
requirements. TO receive HDM, an individual must receive another CCSP service.
Respite Care (RC)
Non-skilled, temporary relief for the primary care givers responsible for
the full-time care of a person who is frail and/or impaired.
Personal Support Services (PSS)
A range of supportive service provided in the individual's home. Personal
Support Services provide light housekeeping and basic personal care.
Home Health Services (HHS)
Skilled services and medical social services provided by a certified,
licensed home health agency. HHS includes nursing services; physical, speech,
and occupational therapies; home health aides; and medical social services to
individuals who meet the homebound criteria.
Adult Day Health (ADH)
A variety of health, therapeutic, and social service activities provided in
a group setting. The services promote an individual's medical stability and
abilities for self-care. Services include nursing care; occupational, speech,
and physical therapies; personal care; dietary services; and social work
services.
Alternative Living Services (ALS)
State licensed residential settings that provide 24-hour supervision and
health-related support services to persons unable to remain in their homes.
Personal care homes enrolled in the ALS program are "home like."
Residents must be able to move about with minimal human assistance and respond
to emergency.
Eligible persons may receive a combination of CCSP services.
To achieve cost-effective service delivery in CCSP, case managers seek needed

How do I know if CCSP is best for me?
"The assessment team completes an assessment of your health needs. The team
involves you, your physician, and your family in the assessment process and
recommends a care plan to you. If you wish to participate in the program, a case
manager arranges services and evaluates your condition on a regular basis to
make appropriate changes in services."
I am not a Medicaid recipient, but I have
limited income and assets and need services. Am I eligible to participate in
CCSP?

eligibility."
How much do CCSP services cost?
"Some individuals pay a fee for services, while others do not."
How long will I be eligible to receive CCSP?
"As long as you meet eligibility and service criteria. However, you may
